FamiliarNote

What helps, carried to the next caregiver.

When a caregiver changes, the new one starts without what the last one knew.

What helps a person is often spread across notes and conversations. The incoming caregiver pieces it together, and the person may have to explain their routines again. FamiliarNote gives the next caregiver a short handoff of reviewed points, each one linked to the entries behind it.

The demo uses a made-up care team and needs no account. Sign in is for invited teams, who use fictional information only for now.

What changed since v1

What Jordan reads before covering for Sam, from the demo's story about Alex.

  • Changed

Alex now chooses a short walk after lunch, about 15 minutes, instead of the park walk before lunch.

Before (v1), under What helps now: Walk to the park before lunch, about 30 minutes. Alex sits on the bench by the pond.

How it works

  1. SamCaregiverCaregivers write short entries about what they saw and what the person said.
  2. MorganCoordinatorEntries become suggested points. AI can suggest them, or the reviewer writes them by hand.
  3. MorganCoordinatorA designated reviewer checks each point against its entries and approves it.
  4. JordanCaregiverThe incoming caregiver reads the handoff, asks about a point and marks it read.

What it does not do

It does not diagnose, give treatment advice or say how someone feels. AI only suggests; a person approves every point. The product works with AI switched off.
